Please complete the pre-enrollment form below:

Filling out the electronic pre-enrollment form starts the enrollment process at our program. Once the pre-enrollment form is submitted and the one time enrollment fee of $70 is paid, the participant’s registration with our program is secured for 18 months.  An invoice will be emailed  for the required amount due for attending, based on the options you choose  on the pre-enrollment form. Invoicing for our program is done on a monthly basis and must be paid in full the Friday before the first Monday of the month, in order for individuals to participate in the month's activities.

Please keep in mind the fees only covers general programming, supplies, basic community outings like bowling, movies, etc. that cost under $8 per person. Any specialty community outings costing more will be the responsibility of the individual should they choose to participate. Notification will go out in advance of all specialty community outings requiring extra funding.

Please download the required INTAKE QUESTIONNAIRE and email to exploreadl@trustedparents.org or mail it to Explore ADL c/o Trusted Parents, P.O. Box 480688 Charlotte, NC 28269 as soon as possible. Additional documents needing a signature will be emailed after enrollment has been secured and completed.
